Registered and Licensed Nurses helping your family organize records and care details for an ALTCS application.
ALTCS (the Arizona Long Term Care System) is the state program administered by AHCCCS that provides long-term care coverage for eligible Arizona residents. Preparing an application involves assembling extensive clinical records from multiple physicians, hospitals, and clinics, along with documenting the daily functional support your loved one requires.
Our ALTCS Records and Readiness Navigation is an optional, private-pay service. Registered and Licensed Nurses at Nurse Navigators assist families in gathering, organizing, and clarifying care documentation so you feel thoroughly prepared.
Written fee schedules are provided to your family in writing prior to any service agreement. We offer three tiers of support depending on your family's needs.
A licensed nurse reviews your existing records, answers procedural preparation questions, and provides a customized checklist of missing documentation.
Focused review of records in hand, gap analysis, and tailored preparation roadmap.
Under your written authorization, we request medical records from up to two healthcare providers, track fulfillment, organize documents chronologically with an index, and draft a factual daily care-needs summary reviewed with your family.
Chronological binder organization, medical release coordination, and functional needs synthesis.
Comprehensive records organization combined with an in-person home visit by a Registered Nurse. The nurse evaluates functional mobility, daily living tasks, and home safety, providing a professional nursing assessment for your family's records packet.
In-person visits across Greater Phoenix (Maricopa County); remote records navigation statewide.
Your family retains full possession of your completed packet and decides independently what to submit and when.
You are never required to hire an outside service to apply for ALTCS. Families may apply directly and at no cost through AHCCCS, online via Health-e-Arizona Plus (healthearizonaplus.gov), or at local AHCCCS and Department of Economic Security (DES) offices.
Free assistance is also available through your local Area Agency on Aging (such as Area Agency on Aging Region One in Maricopa County at 602-264-4357) and non-profit legal aid organizations. Many families complete their applications independently. We are available if you would like professional nurse assistance with record gathering and functional care documentation.
