Important: Nurse Navigators provides direct hands-on personal care, ADL support, and clinical care navigation under licensed RN oversight on a private-pay basis. We are not an ADHS Chapter 9 Medicare-certified home health agency and do not perform physician-ordered skilled procedures (such as sterile surgical wound care or IV therapy) or provide 24/7 emergency response (call 911 for emergencies).

- Operating as a Medicare-certified home health agency
