What exactly does Nurse Navigators do?
Nurse Navigators is an Arizona-based care navigation and compliance consulting firm licensed RN care navigation. We serve two distinct audiences: families who need help coordinating, advocating for, and managing complex care — and facilities that need compliance auditing, regulatory guidance, and staff training.
For families, we're the person in the room who knows what's being said, catches what's being missed, and coordinates the full picture. For facilities, we're the auditor who inspects before the state does.
Is Nurse Navigators a home health agency?
No. Nurse Navigators is not a home health agency, hospice, skilled nursing facility, or insurance-based care provider. We operate as a private-pay care navigation and consulting practice.
Our services include personal care assistance, companionship, coordination, and advocacy — along with RN-level oversight and clinical consulting for facilities. We do not provide skilled nursing procedures, IV therapy, wound care nursing, or Medicare/Medicaid-billed services.
Who are the navigators? What are their credentials?
Our navigators are licensed Registered Nurses and experienced care professionals who have worked bedside, in clinical settings, and inside the compliance system itself. This isn't a staffing agency model — we are clinical professionals who've carried the same care loads we now navigate.
Every engagement involves licensed RN oversight. Specific credential details are available on request.
What areas of Arizona do you serve?
Our primary service area is Greater Phoenix, Arizona, including the East Valley (Mesa, Chandler, Gilbert, Tempe), Scottsdale, and surrounding communities.
Compliance consulting and legal nurse consulting engagements are available statewide and remotely. Contact us if you're outside the Phoenix metro — many services can be adapted.

We complement the medical system — we don't replace it.
Can you help with hospital discharge planning?
Yes — this is one of our most common engagements. Our Hospital-to-Home Bridge service manages the entire discharge coordination process: reviewing the discharge plan, communicating with the care team, arranging post-acute services, and conducting 30-day follow-up to prevent readmission.
If your loved one is being discharged and you don't understand the plan or aren't confident in the next step — call us immediately. We can often engage same-day.
Do you accept insurance or Medicare?
No. Nurse Navigators is a private-pay service. We do not accept Medicare, Medicaid, or any insurance plan for payment.
This is intentional — it keeps us working exclusively for you, not a payer. Some clients use HSA/FSA funds for applicable services. We provide detailed invoices for your records.
What does care navigation cost?
Every family and every facility is different, so we provide custom quotes based on your specific situation. Pricing depends on the type of service, duration, and level of care needed.
We offer services across personal care, companionship, RN consultation, concierge care, compliance auditing, and more. Call us or use our intake form to describe your situation — we will give you an honest, straightforward quote the same day.

How quickly can you start?
For urgent care navigation requests (hospital discharge, crisis situations), we can often engage same-day. Standard intake is completed within one business day.
Compliance consulting engagements are typically scheduled within 5—10 business days, depending on facility availability and audit scope.
What types of facilities do you work with?
We primarily serve Assisted Living Facilities (ALFs) and Skilled Nursing Facilities (SNFs) in Arizona. We also work with adult day programs, memory care communities, and independent living facilities that have care components.
Our compliance consulting is built around Arizona ADHS regulatory standards and CMS requirements.
What's the difference between a Solstice Audit and a Gap Analysis?
Solstice Audit: A comprehensive, full-facility compliance audit covering documentation, care planning, medication management, staff records, and physical plant — benchmarked against current ADHS standards. Think of it as a mock survey. Delivers a detailed written report with findings, risk levels, and corrective action recommendations.
Gap Analysis: A targeted, faster assessment focused on a specific regulatory domain — medication management, staffing ratios, documentation practices, or a post-deficiency review. Scope is defined in advance.
