HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment to Your Privacy
Nurse Navigators operates as a care navigation and consulting practice. To the extent NNP handles Protected Health Information (PHI) in connection with care coordination, we implement safeguards consistent with HIPAA requirements. NNP navigators do not function as treating providers — they serve as coordinators, advocates, and consultants. Specific HIPAA obligations depend on the nature of each engagement and any applicable Business Associate Agreements in effect.

We may use and disclose your PHI for the following purposes:
- Treatment and Care Coordination: To coordinate your care with physicians, specialists, and care facilities.
- Healthcare Operations: To run our practice, improve your care, and contact you when necessary.

You have the right to:
- Inspect and request a copy of your medical records and PHI.
- Request an amendment to your records if you believe they are incorrect or incomplete.
- Request an accounting of disclosures of your PHI.
