Nurse Navigators Pro provides nursing oversight, advocacy, and care coordination. We do not replace your primary care physician or specialists. Our services are non-diagnostic. All medical decisions regarding prescription medications, surgical interventions, and diagnoses must be made by a licensed physician. In the event of a medical emergency, you agree to call 911 immediately.
Engaging our services constitutes acceptance of our Client Service Agreement. We operate as a private-pay service. Invoices are generated upon service delivery or per the agreed retainer schedule. Payment is due upon receipt. We do not bill Medicare, Medicaid, or private insurance directly, though we may provide documentation to assist you in seeking reimbursement.
To the maximum extent permitted by applicable law, Nurse Navigators Pro shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, or any loss of data, use, goodwill, or other intangible losses, resulting from (a) your access to or use of or inability to access or use the services; (b) any conduct or content of any third party on the services.
Any dispute, controversy, or claim arising out of or relating to these Terms or the breach, termination, enforcement, interpretation, or validity thereof, including the determination of the scope or applicability of this agreement to arbitrate, shall be determined by arbitration in Maricopa County, Arizona, before one arbitrator.
We shall not be liable for any failure to perform our obligations where such failure results from any cause beyond our reasonable control, including, without limitation, mechanical, electronic, or communications failure or degradation.
Nurse Navigators Pro uses SMS messaging to provide timely clinical advocacy. This includes, but is not limited to: confirming nursing visits, alerting families of status changes, coordinating with facility staff, and billing notifications.
Message frequency varies based on your active care needs. During a crisis or transition of care, frequency may increase to ensure safety. You may receive 1-5 messages per week during active service periods.
Message and data rates may apply depending on your carrier plan. Nurse Navigators Pro is not responsible for any messaging or data charges incurred by you. Carriers (T-Mobile, AT&T, Verizon, etc.) are not liable for delayed or undelivered messages.
