Nurse Navigators Pro ("Company," "we," "us," "our") operates as a clinical advocacy and healthcare coordination service. By accessing our website, services, or engaging with our staff, you acknowledge that you have read and understood this Privacy Policy. This policy applies to all information collected through our website, mobile applications, and direct service interactions.

2. Information We Collect

We collect information necessary to provide high-level clinical services. The types of data we collect include:

  • Identity Data: First name, last name, date of birth, and relationship to the patient.
  • Contact Data: Billing address, email address, and telephone numbers.
  • Health Data (PHI): Medical history, diagnosis, medication lists, and care plans. Note: Protected Health Information (PHI) is collected exclusively through secure, HIPAA-compliant channels and never via public web forms.
  • Technical Data: Internet Protocol (IP) address, browser type and version, time zone setting, and operating system used to access our digital platforms.

3. Strict No-Sharing Protocol (Carrier Compliance)

NON-NEGOTIABLE PRIVACY STANDARDS: We do not sell, trade, rent, or otherwise transfer your Personally Identifiable Information (PII) or mobile phone numbers to outside parties for marketing purposes.

SMS Consent is not shared with third parties. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

4. Purpose of Data Usage

We use your data solely for the following operational purposes:

  • To provide clinical assessments, advocacy, and care management services.
  • To process payments and manage billing accounts.
  • To communicate regarding appointment scheduling and care plan updates.
  • To comply with legal obligations, including Arizona state regulations and federal healthcare laws.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Our Legal Duty

We are required by applicable federal and state law to maintain the privacy of your health information.   • Healthcare Operations: We may use and disclose your health information in connection with our healthcare operations, including quality assessment and improvement activities, reviewing the competence or qualifications of healthcare professionals, evaluating practitioner and provider performance, and conducting training programs.

3. Your Authorization

In addition to our use of your health information for treatment, payment, or healthcare operations, you may give us written authorization to use your health information or to disclose it to anyone for any purpose. If you give us an authorization, you may revoke it in writing at any time.

4. Business Associates

We may disclose your health information to contractors, agents, and other business associates who need the information in order to assist us with obtaining payment or carrying out our business operations. We obtain a written agreement from each business associate that contains terms requiring them to protect the confidentiality of your health information.

Terms of Service

1. Clinical Scope & Non-Medical Disclaimer

Nurse Navigators Pro provides nursing oversight, advocacy, and care coordination. We do not replace your primary care physician or specialists. Our services are non-diagnostic. All medical decisions regarding prescription medications, surgical interventions, and diagnoses must be made by a licensed physician. In the event of a medical emergency, you agree to call 911 immediately.

2. Service Agreement & Billing

Engaging our services constitutes acceptance of our Client Service Agreement. We operate as a private-pay service. Invoices are generated upon service delivery or per the agreed retainer schedule. Payment is due upon receipt. We do not bill Medicare, Medicaid, or private insurance directly, though we may provide documentation to assist you in seeking reimbursement.

3. Limitation of Liability

To the maximum extent permitted by applicable law, Nurse Navigators Pro shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, or any loss of data, use, goodwill, or other intangible losses, resulting from (a) your access to or use of or inability to access or use the services; (b) any conduct or content of any third party on the services.

4. Dispute Resolution & Arbitration

Any dispute, controversy, or claim arising out of or relating to these Terms or the breach, termination, enforcement, interpretation, or validity thereof, including the determination of the scope or applicability of this agreement to arbitrate, shall be determined by arbitration in Maricopa County, Arizona, before one arbitrator.

5. Force Majeure

We shall not be liable for any failure to perform our obligations where such failure results from any cause beyond our reasonable control, including, without limitation, mechanical, electronic, or communications failure or degradation.

SMS / Text Message Terms & Conditions

Explicit Opt-In Consent: By explicitly providing your phone number to Nurse Navigators Pro via our secure intake forms, or by texting us at (520) 607-2766, you consent to receive text messages regarding your care coordination, appointment reminders, and urgent clinical updates.

1. Program Description

Nurse Navigators Pro uses SMS messaging to provide timely clinical advocacy. This includes, but is not limited to: confirming nursing visits, alerting families of status changes, coordinating with facility staff, and billing notifications.

2. Message Frequency

Message frequency varies based on your active care needs. During a crisis or transition of care, frequency may increase to ensure safety. You may receive 1-5 messages per week during active service periods.

3. Cost and Carrier Liability

Message and data rates may apply depending on your carrier plan. Nurse Navigators Pro is not responsible for any messaging or data charges incurred by you. Carriers (T-Mobile, AT&T, Verizon, etc.) are not liable for delayed or undelivered messages.

4. Opt-Out Protocol

You may revoke consent at any time through the following methods:

  • Reply STOP to any message to unsubscribe immediately.
  • Reply HELP for assistance or technical support.
  • Email our compliance team at contact@nursenavigatorspro.com.
