THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
LISTERKEYS LLC, dba Nurse Navigators is a Covered Entity (or Business Associate, depending on the engagement) under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We are required by law to maintain the privacy of your Protected Health Information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to abide by the terms of this Notice currently in effect.

How We Use and Disclose Your Health Information

Treatment

We may use and disclose your PHI to coordinate your care with other healthcare providers, specialists, hospitals, and care facilities involved in your treatment. For example, we may share your health history with a specialist we are advocating for you to see.

Payment

We are a private-pay practice and do not bill Medicare, Medicaid, or insurance. We may use and disclose your PHI for our own billing and payment purposes, such as preparing invoices, processing payments you make directly to us, and maintaining financial records for services we provide to you.

Healthcare Operations

We may use and disclose your PHI for our internal operations, including quality assessment, training of staff, and administrative functions required to run our practice.

Other Permitted Disclosures

  • As Required by Law — We will disclose PHI when required by federal, state, or local law
  • Public Health Activities — To prevent or control disease, injury, or disability as required by public health authorities
  • Abuse or Neglect — To report suspected abuse, neglect, or domestic violence to appropriate authorities
  • Law Enforcement — In response to a court order, warrant, or law enforcement request
  • Emergency Situations — To prevent a serious threat to your health and safety or to the public

Your Rights Regarding Your Health Information

Right to Access

You have the right to inspect and receive a copy of your PHI that we maintain in our records.

Right to Amend

You have the right to request that we correct any PHI you believe is inaccurate or incomplete.

Right to Restrict

You have the right to request a restriction on certain uses and disclosures of your PHI.

Right to an Accounting

You have the right to request a list of disclosures we have made of your PHI in the past six years.

Right to a Paper Copy

You have the right to receive a paper copy of this Notice at any time, even if you agreed to receive it electronically.

Right to File a Complaint

You have the right to file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.

Our Duties

We are required by law to maintain the privacy of your PHI and to provide you with this Notice. We are required to abide by the terms of the Notice currently in effect. We reserve the right to change our privacy practices and the terms of this Notice at any time, provided the changes are permitted by law. We will make available to you the current Notice via our website and upon request.

How to Exercise Your Rights or File a Complaint

To exercise any of the rights described above, or to file a complaint about our privacy practices, please contact our Privacy Officer. We will not retaliate against you for filing a complaint. You may also file a complaint directly with the Secretary of Health and Human Services at hhs.gov/hipaa/filing-a-complaint.
