1. About Nurse Navigators

LISTERKEYS LLC, dba Nurse Navigators ("NNP") is a private care navigation and consulting company registered in Arizona. We provide direct hands-on personal care and Activities of Daily Living (ADL) assistance, licensed Registered Nurse care navigation, clinical advocacy, caregiver support, and facility compliance consulting on a private-pay concierge basis. NNP is not an ADHS Chapter 9 Medicare/Medicaid-certified home health agency, hospice, or skilled nursing facility.

2. Scope of Services

NNP delivers in-home personal care and client advocacy supervised by licensed Registered Nurses and qualified care professionals. Our services include, but are not limited to:

• Direct, hands-on personal care and Activities of Daily Living (ADL) support (including assisted bathing, showers, bed baths, personal hygiene, incontinence care, dressing, grooming, safe transfers, Hoyer lift assistance, and feeding support)
• Care navigation and family advocacy (coordinating with physicians, specialists, and hospital discharge teams)
• Medication reminders and care routine management
• Hospital-to-home transitional care and comfort care planning
• Arizona Assisted Living Facility (ALF) compliance consulting and Article 8 pre-survey audits
• Caregiver training, clinical education, and family consulting

Clinical Boundaries & Non-Covered Services: NNP does not diagnose medical conditions, prescribe medication, or bill Medicare, Medicaid, or commercial insurance. NNP does not perform physician-ordered skilled nursing clinical procedures requiring specialized home health orders (such as sterile surgical wound debridement, IV infusions/central line maintenance, or catheter placement). Emergency medical situations require immediate 911 contact.

3. Website Use

You may use our website for lawful purposes only. You agree not to:

• Submit false, misleading, or fraudulent information through our contact forms
• Attempt to gain unauthorized access to our systems or client data
• Transmit malware, spam, or other harmful content through our site
• Reproduce, copy, or redistribute our content without written permission
