1. About Nurse Navigators
Nurse Navigators ("Nurse Navigators", "we", "us", or "our") is an independent, private-pay Registered and Licensed Nurse care navigation and clinical consulting practice registered in Arizona. We provide direct hands-on personal care and Activities of Daily Living (ADL) assistance, care navigation, clinical advocacy, caregiver support, and facility compliance consulting on a direct private-pay concierge basis.
2. Scope of Services
Nurse Navigators delivers in-home personal care and client advocacy supervised by Registered and Licensed Nurses and qualified care professionals. Our services include, but are not limited to:
- Direct, hands-on personal care and Activities of Daily Living (ADL) support (including assisted bathing, showers, bed baths, personal hygiene, incontinence care, dressing, grooming, safe transfers, mechanical lift assistance, and feeding support)
- Care navigation and family advocacy (coordinating with physicians, specialists, and hospital discharge teams)
- Medication reminders and care routine management
- Hospital-to-home transitional care and comfort care planning
- Arizona Assisted Living Facility (ALF) compliance consulting and Article 8 pre-survey audits
- Caregiver training, clinical education, and family consulting
Clinical Parameters: Nurse Navigators provides care navigation, in-home care assessments, and clinical consulting on a direct private-pay basis. Our services complement your existing healthcare team and do not replace the medical diagnosis or treatment plan of your attending physician. Emergency medical situations require immediate 911 contact.
